How to Apply
Initial Steps for Prospective Applicants
1. Initial Inquiry: Download the Grant Proposal Summary sheet from www.LFCPF.org.
2. Email the completed summary to info@LFCPF.org.
3. Invitation to Apply: LFCPF staff will contact applicants whose summaries meet basic requirements and invite them to submit a full application.
4. Full Application Submission: Prepare and submit a typed application including:
Proposal Summary
Grant Narrative
Program/Project Budget
Required Attachments (see checklist below)

Submitting a Full Grant Application
Applicants approved to move forward will be contacted by LFPCF staff with an invitation to submit a formal grant request. The completed full grant application must be typed and include:

• Program/Project Budget
• Required Attachments

Application Deadlines
Applications are accepted throughout the end of November.
Grant Review Process
1. Initial Screening: The executive director reviews all submissions in order of receipt.
2. Eligibility Check: The executive director and board president assess whether applicants meet eligibility guidelines.
3. Ineligible applicants will not be notified.
4. Eligible applications are forwarded to the selection committee.
5. Committee Review: The selection committee, appointed from board members, makes unanimous decisions on grant recipients, prioritizing applicants with the greatest financial need and highest community impact.
Award Notification
Final notifications are sent within six to eight weeks after receiving the full application.
Ineligibility Clause
Immediate family members of the Perry Board of Directors and the Grant Awards Selection Committee are not eligible to receive grants or scholarships. All board and committee members must confirm their relationship status with applicants; these declarations are recorded in official minutes.
Grant Payment and Reporting
• Payments: Grants are paid directly to recipient organizations at the beginning of the project. The board may opt to release half of the funds upfront, requiring an interim report before releasing the remaining amount.
• Reporting: Grantees must provide both fiscal and narrative reports detailing fund usage and project impact. An interim report is due midway through the project, and a final report is required at completion. Reporting requirements are specified in the award letter.
• Use of Funds: Grants may only be used for the purposes outlined in the proposal.
Required Attachments Checklist
- Names and qualifications of program staff
- Detailed project budget and narrative, including:
- Program Budget Worksheet
- Income sources and expenditures
- Other funding requests (pending and approved)
Financial Statements
Annual operating budget (budgeted and actual amounts)
Balance sheets and cash flow statements
Statements of support, revenue, and expenses for the previous and current year
Most recent IRS 990 Form
List of board members, including contact information, occupations, term of office, and rotation schedule
Board statement authorizing the grant request and agreeing to implement the project (see attached Board Resolution and Agreement forms)
IRS 501(c)(3) determination notice or, if applying through a fiscal agent, their 501(c)(3) documentation and letter of agreement
Marketing collateral (e.g., brochures)
